Covid-19 Community Outreach Vaccination Programme at UTP Chancellor Hall - Second Dose of Vaccination on 21 August 2021

 

Ladies and Gentlemen,

 

For those who will be receiving their second dose of vaccination on 21 August 2021, please do the following steps:

1. Please read the attached Joining Instruction for Staff.
2. Please bring along the Covid-19 Vaccination Consent Form and Covid-19 Vaccination Card.
3. Please check your MySejahtera for your vaccination appointment time regularly should there be any changes. If your MySejahtera has not been updated for the second dose, please come on the said date as Kementerian Kesihatan Malaysia personnel will update your vaccination status during registration. If you are travelling cross state/district (except those from the EMCO areas), you may show the police the details on your MySejahtera.

 

Please wear your mask and practise social distancing at all times.
