Covid-19 Community Outreach Vaccination Programme at UTP Chancellor Hall on 31 July 2021

 

Ladies and Gentlemen,

 

For those who have registered for the vaccination on 31 July 2021, please do the following steps:

 

  1. Please read the attached Joining Instruction for Staff.
  2. Please print and bring along 2 copies of the attached Covid-19 Vaccination Consent Form.  Fill in all required details except the witness signature, which will be signed by the medical officer at UTP Chancellor Hall on 31 July 2021. 
  3. Please check your MySejahtera for your vaccination appointment.  If your MySejahtera has not been updated, you may refer to the attached letter from Pejabat Kesihatan Daerah Perak Tengah for your appointment details.  If you are travelling cross state/district (except those from the EMCO areas), you may show the police the details on your MySejahtera app or the attached letter from Pejabat Kesihatan Daerah Perak Tengah to proof your vaccination appointment.

 

Please wear your mask and practise social distancing at all times.
